2025-2026 Before and After Care Registration Form
2025-2026 School Year
Please complete the Before and After Care registration form and submit it to either the BA Care Site Supervisor or Elementary School Secretary. Once the registration form is received at the BA Care office, you will be notified of enrollment or if your child is placed on a waiting list.

*Please note- spaces may be limited due to staffing and registration is first come, first serve. Students are not considered registered until confirmed by the Before and After Care/ Preschool office.
Checks or money orders should be made payable to O'Fallon District #90. Registration papers and Parent handbook are available at each school office. The site directors are available during program hours 7:00 am until 8:45 am and after school 3:00 pm until 6:00 pm.

Before and/or After Care Counselor Employment
District #90 offers Before and/or After Care in all 5 Elementary Buildings. Please complete the linked application for employment with Before and/or After Care as a Caregiver/Counselor. You will be contacted once the application is received with more information.

Payments may be made by money order, check, or online with credit card.
Checks should be made payable to O’Fallon District 90. Receipts will be given for payments.
Online Revtrak payments may be made once registration is complete. Families choosing the Monthly Budget Billing Option will also be able to make online payments through Skyward after the first month.
