Note: Once submitted, you will receive two emails from Jotform. One email is simply a confirmation that the form was submitted successfully. The other email will have the completed form attached as a PDF document. This document is required to be uploaded/attached when you submit your volunteer application in Step 3.
For best results, please complete the waiver form on a desktop, laptop or tablet device. Some users have experienced an issue with downloading the form when using a smartphone/mobile device.
When downloading the form, please select "Save As" and rename the file with your first and last name. Example: John Doe Waiver.pdf
